What Does Sinus Elevation Involve?

During your sinus elevation procedure, Dr. Samadian and his highly trained staff will make a tiny incision into your gums. Next, they’ll fashion the exposed jawbone to form a moveable flap using ultrasonic devices, which will be gently driven upward into your sinus cavity using water pressure and vibrations.

This “lifting” procedure results in an empty space that can then be filled with bone graft material. In some instances, Dr. Samadian may even place your dental implant right away, or it may be prudent to wait six or more months for implant placement. Before your procedure, Dr. Samadian will discuss your timeline with you in detail, so you’ll know exactly what to expect.

Lateral and Crestal Sinus Lifts

There are two ways to reach the sinus floor. In a crestal (internal) sinus lift, the membrane is raised gently through the implant site itself and a small amount of graft placed beneath it, often with the implant placed at the same visit. It suits cases that need only a few millimeters of extra height.

In a lateral window sinus lift, a small window is made in the side of the upper jaw, the sinus membrane is lifted, and the space beneath is filled with bone graft. It is used where much more height is needed, and the implant is placed either at the same time or after the graft has matured.

Materials

Sinus grafts typically use donor or animal-derived bone, sometimes mixed with your own PRF. Where a large amount of bone is needed, rhBMP-2, which is FDA-approved for sinus augmentation, may be used. See growth factors and PRF.

Recovery and Risks

Expect swelling and sometimes bruising of the cheek, a little nasal bleeding, and congestion for a few days. For about two weeks you avoid blowing your nose, sneeze with your mouth open, and avoid flying or diving unless cleared. Risks include a tear in the sinus membrane (usually repaired during the procedure), infection, sinusitis, and partial loss of the graft. Existing sinus problems are assessed on the CBCT scan beforehand.

How long does a sinus lift take to heal?

The gum heals in about two weeks; the graft takes several months to mature before an implant can be placed in it. With a crestal lift the implant may go in at the same visit.

Is a sinus lift painful?

It is done under local anesthetic, with sedation available. Afterwards most patients describe pressure and swelling more than pain, managed with medication.

What is the difference between a crestal and a lateral sinus lift?

A crestal lift raises the sinus floor through the implant site and adds a few millimeters of bone. A lateral lift works through a small window in the side of the jaw and can add much more.

Can I fly after a sinus lift?

Not straight away. Pressure changes can disturb the graft; you will be told when it is safe, usually after the first couple of weeks.
